SERVICE AGREEMENT

This Service Agreement (“Agreement”) is entered into between Live by Choice Coaching Academy (“the Company”) and the contracting individual or organization (“the Client”).

1. SERVICES

The Company agrees to provide professional Coaching, Consulting, Facilitation, and/or Personal Breakthrough Services (“Services”) as agreed with the Client.

2. TERM OF AGREEMENT

2.1 Coaching Services
The initial coaching commitment is three (3) months from the date of this Agreement or the first scheduled session, whichever occurs first. Thereafter, services are reviewed on a quarterly basis.

2.2 Consulting, Facilitation & Personal Breakthrough Services
These services shall commence on an agreed start date and end upon completion or expiry of the agreed service period.

3. CLIENT RESPONSIBILITIES

The Client agrees to:

Cooperate fully with the Company

Attend scheduled sessions

Provide at least 48 hours’ notice for cancellations or rescheduling
Sessions cancelled with less than 48 hours’ notice, or missed without notice, are deemed delivered and may be charged in full according to the Company’s fee schedule. Initiating and attending sessions is the Client’s responsibility.

4. FEES, INVOICING & PAYMENT

4.1 Fees

Consulting Fees: Invoiced monthly in arrears

Coaching Retainer Fees: Payable quarterly in advance

Personal Breakthrough Fees: 50% payable on signing; balance payable upon completion

4.2 Invoices & Payment Terms
Invoices are payable within 14 days of issue. The Company may suspend services if payments are overdue and reserves the right to charge interest and recover collection costs on late payments.

4.3 Expenses
The Client shall reimburse all reasonable expenses incurred in connection with the Services, including approved travel and materials. Extraordinary expenses require prior Client approval.

4.4 Prepaid Travel
Where the Client arranges travel or accommodation, confirmations must be provided at least 10 days prior to the service date.

5. CHANGES & EXTENSIONS

Any changes to the scope of Services must be agreed upon in writing by both parties. If services are extended due to Client-related delays or changes, additional fees may apply. If the Service is frustrated or terminated due to the Client, the Company is entitled to payment for all work completed and resources committed.

6. INDEPENDENT CONTRACTOR

The Company acts as an independent contractor and not as an employee, partner, or agent of the Client.

7. CONFIDENTIALITY

All coaching and related discussions are strictly confidential. The Company will not disclose any Client information or the existence of the coaching relationship without the Client’s consent, except as required by law.

8. GENERAL

Both parties agree to act in good faith and to resolve any disputes amicably where possible, without prejudice to their legal rights.

Client Acceptance
By signing this Agreement, the Client confirms acceptance of all terms and conditions herein.
